How to Calculate Corrected Calcium
To calculate corrected calcium, enter the measured total serum calcium and serum albumin from the same blood sample. The commonly used simplified albumin-correction equation is:

Corrected Calcium Calculation Example
Suppose a laboratory report shows total calcium of 8.0 mg/dL and albumin of 2.5 g/dL.

Whether any result is clinically normal depends on the reporting laboratory, the patient, and whether the equation is appropriate.
Why Correct Calcium for Albumin?
Calcium circulates in three main forms. Roughly half is ionized, a substantial portion is bound to albumin, and the remainder is complexed with anions such as phosphate or citrate. The ionized fraction is biologically active in nerve signaling, muscle contraction, coagulation, and cardiac electrical activity.

When albumin falls, measured total calcium can fall because less calcium is protein-bound—even when ionized calcium has not fallen to the same degree. The calculation attempts to express total calcium at a reference albumin. It does not reconstruct the patient's true ionized calcium.
How to Interpret the Result Safely
Compare the measured and calculated values with the reference interval on the same laboratory report. Adult intervals often sit near 8.5–10.5 mg/dL (about 2.12–2.62 mmol/L), but assays and laboratories differ. This page therefore lets you enter the laboratory's own lower and upper limits.

An estimate outside the laboratory interval does not identify the cause. Clinical assessment may consider symptoms, repeat testing, kidney function, magnesium, phosphate, parathyroid hormone, vitamin D, medications, hydration, and acid-base status.
When Corrected Calcium May Be Unreliable
The formula is common, but common does not mean universally accurate. Direct ionized calcium is often more informative when calcium status will change care.
- Advanced CKD or dialysis: albumin adjustment can overcorrect and misclassify calcium status. See the GFR calculator for a separate kidney-function estimate.
- Critical illness: altered protein binding, lactate, phosphate, citrate, and rapidly changing physiology weaken the estimate.
- Acid-base disturbance: pH changes calcium binding to albumin. The formula contains no pH input; an ABG calculator answers a different question and does not repair this limitation.
- Severe hypoalbuminemia: recent evidence found greater misclassification when albumin was below 30 g/L (3.0 g/dL).
- High albumin or abnormal proteins: the equation was not designed as a universal correction for every protein state.
- Pregnancy, pediatrics, or major transfusion: use population- and situation-specific clinical interpretation rather than a generic online range.
Common Calcium Correction Errors
Mixing units. Albumin 25 g/L equals 2.5 g/dL; entering 25 in a g/dL field makes the result meaningless.
Treating the estimate as ionized calcium. The two values are not interchangeable.
Using a generic normal range. Reference intervals vary by laboratory and assay.
Ignoring the clinical setting. CKD, dialysis, critical illness, pH disturbance, and citrate exposure matter.
Using mismatched samples. Calcium and albumin should come from the same collection context.
Rounding too early. Keep precision through the calculation, then round the final value.

How do you calculate corrected calcium?
In US units, add 0.8 × (4.0 − albumin in g/dL) to measured total calcium in mg/dL. In SI units, add 0.02 × (40 − albumin in g/L) to measured calcium in mmol/L.
Why do we calculate corrected calcium?
Low albumin can lower total calcium because less calcium is protein-bound. The calculation estimates an albumin-adjusted total, but it does not directly measure active ionized calcium.
Is corrected calcium accurate in kidney disease?
It may be inaccurate, particularly in advanced CKD and dialysis. Published studies report poor agreement with ionized calcium in these populations. Ask whether direct ionized calcium is appropriate.
What is the difference between corrected and ionized calcium?
Corrected calcium is calculated from total calcium and albumin. Ionized calcium is a direct laboratory measurement of the free fraction and is affected by sample handling and pH.
Should calcium be corrected when albumin is high?
Do not assume the simplified equation is validated for every high-albumin state. Its original purpose and performance depend on the population and laboratory method. Use the lab's guidance or direct ionized calcium when accuracy matters.

The calculator reproduces the widely used simplified albumin-correction equation. The original 1973 Payne paper derived a laboratory-specific adjustment from 200 specimens and reported a different regression coefficient than the later simplified 0.8 formula commonly used online. This distinction is often omitted.
