How to Calculate Semaglutide Dosage from a Label

A semaglutide dosage calculation needs two facts: the dose already prescribed in milligrams (mg) and the liquid concentration in milligrams per milliliter (mg/mL). It should never start with a guessed dose.

Suppose a prescription says 0.25 mg and the vial label says 2.5 mg/mL. Divide 0.25 by 2.5. The calculated volume is 0.10 mL.

How to Read the Concentration on a Compounded Vial

Compounded semaglutide labels may express strength in different ways. That makes careful reading essential. A compounded semaglutide dosage calculator is only as accurate as the label values entered.

Do not reverse-engineer a dose. Ask the pharmacy for mg, mg/mL, mL, and the syringe type.

Check every refill. A new vial can have a different concentration even when the prescribed milligram dose stays the same. Recalculate only after reading the new label and confirming it with the pharmacy.

Milligrams, Milliliters, and Syringe “Units” Are Different

U-100Syringe marks

On a U-100 insulin syringe, 100 marks equal 1 mL. These marks are volume, not milligrams.

For the worked example above, 0.10 mL lines up with 10 marks on a U-100 syringe. But 10 marks can contain a different number of milligrams when the concentration changes. It is unsafe to copy a unit number from another person, another vial, or an earlier refill.

Do not apply this conversion to a U-40 syringe or another device. The pharmacy should supply the right syringe and show the intended mark.

Is Semaglutide Dosage Calculated by Weight?

No simple body-weight formula is used by this calculator. Search phrases such as “semaglutide dosage calculator by weight” can be misleading. FDA-approved semaglutide products follow product- and indication-specific instructions rather than a universal mg-per-kilogram equation.

A prescriber considers the approved product label, treatment goal, medical history, other medicines, response, and side effects. Body weight may help determine whether a medicine is appropriate for a condition, but it does not let a website choose an injection dose.

Approved Products and Compounded Semaglutide Are Not Interchangeable

Semaglutide is a glucagon-like peptide-1 (GLP-1) receptor agonist. Approved products have product-specific labels, dosage forms, instructions, strengths, and uses. A compounded product is not an FDA-approved generic version.

Do not use this tool to transfer a dose between Ozempic, Wegovy, oral semaglutide, or a compounded vial. Contact the prescriber and pharmacist when the product or dosage form changes.

Why Compounded Semaglutide Dosing Errors Happen

The FDA has received reports of dosing errors with compounded injectable semaglutide. Some people took 5 to 20 times the intended dose. Reported problems included confusion between mg, mL, and units; different vial concentrations; and syringes that were too large for the intended volume.

  • Concentration copied incorrectly: “5 mg/2 mL” is not the same as 5 mg/mL.
  • Decimal moved: 0.05 mL and 0.5 mL differ tenfold.
  • Units treated like milligrams: syringe marks measure volume and depend on concentration.
  • Old instructions reused: a refill may use a new concentration.
  • Product identity assumed: salt forms such as semaglutide sodium or acetate differ from the active ingredient in approved products.
If the result looks different from the label directions, stop.

Do not choose the number that seems more likely. Do not inject while waiting for an online answer. Call the dispensing pharmacy and ask for a written confirmation of dose in mg, concentration in mg/mL, volume in mL, and syringe mark.

  • No concentration: vial size alone cannot tell you the correct volume.

Severe or persistent vomiting, severe abdominal pain, fainting, dehydration, confusion, or symptoms of low blood sugar need prompt medical assessment, especially after a possible dosing error.

Is semaglutide dosage calculated by body weight?

Not with a universal mg-per-kg formula. Approved products use product- and indication-specific directions. A prescriber selects a dose using the label, medical history, response, tolerability, and other medicines.

Are semaglutide mg and syringe units the same?

No. Mg measures medication amount. mL measures liquid volume. On a U-100 syringe, 100 marks equal 1 mL, but the mg in that volume depends on the vial concentration.

Can I use the calculator for Ozempic or Wegovy pens?

No manual vial calculation is needed for a prefilled pen. Use the prescribed pen and follow its Instructions for Use. This calculator is not a pen-click guide and does not calculate tablet doses.

What if my compounded semaglutide label is unclear?

Do not guess, mix, or inject it. Ask the dispensing pharmacy to confirm the prescribed mg, mg/mL concentration, exact mL volume, and correct syringe in writing.
